Thank you for choosing First Coast Heart & Vascular Center and for entrusting us with your cardiology health care needs. First Coast Heart & Vascular Center provides you and your family with comprehensive, quality medical care.

Our phone lines are open 8 a.m. – 5 p.m., Monday – Friday, except for holidays.

For medical emergencies or if you are having chest pain, call 9-1-1.

If this is an urgent issue, please call our office. If it is outside of regular business hours our answering service will get in touch with the First Coast Heart & Vascular Center provider on call.

Appointment Policies

Making an Appointment

  • If your issue is urgent, please call our office to request an appointment.

Canceling an Appointment

  • If you need to cancel an appointment, we require 24 hours notice.

Co-pays

  • Co-pays must be paid in full at the time of the visit.

Prescription Refills

For your convenience, we use e-prescribe. For the most efficient service on routine refills, please call your pharmacy directly.

Please allow 48 hours for all prescription refills.

Questions About Your Bill

Your insurance company is the best resource for most billing questions. If you have not done so already, please call the number on your insurance card. If your insurance company cannot resolve the issue, please contact us. A First Coast Heart Vascular Center billing specialist will get back to you.

Release of Health Information 

First Coast Heart and Vascular has partnered with Sharecare to fulfill your requests for records. 

First Coast Heart and Vascular is committed to protecting your medical information. For information about your rights and the obligations you have regarding the use and disclosure of your medical information, please see our Notice of Privacy Practices. 

If you are our patient and would like to request your medical records, please click on the link below to complete your request for medical records. You will be required to provide a valid email address and a government-issued ID.

If you are unable to complete the electronic form above, download and complete the paper form below. The authorization form must be legible and complete. Authorizations that are illegible or incomplete will be returned. Please note the processing of records is faster using the online form above. 

Records are usually available within 10 days from the time the request is received. If you are picking up your medical records in person, please be sure to bring a government-issued ID. To arrange for another individual to pick up the documents for you, please indicate that on the authorization form, and ask them to bring their government-issued ID. 

Only the patient, parent/legal guardian, or the patient’s legal health care representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for healthcare/ Conservatorship.
